What is the CDM Programme?

The Chronic Disease Management (CDM) programme is a structured, HSE-funded programme for patients with long-term conditions. At Kinsealy Medical Centre, we run a comprehensive recall and management programme for patients with diabetes (Type 2), COPD, asthma, hypertension, and cardiovascular disease.

Eligible patients receive two free structured reviews per 12-month period with our nursing team, providing personalised care planning, self-management support, and early detection of complications — all in your local community.

Who is eligible?

• Patients aged 18 and over
• Holding a medical card or GP Visit Card
• With a confirmed diagnosis of Type 2 diabetes, COPD, asthma, hypertension, or cardiovascular disease

How the programme works

You will be invited for two reviews per year. These are carried out by our practice nurses and may include blood pressure checks, blood tests, weight measurement, medication review, and discussion of your overall health and lifestyle. Your GP will be involved where clinical assessment is required.

Prescriptions and the CDM programme

Patients enrolled in the CDM programme must complete a nurse review every 6 months before repeat prescriptions are issued for their chronic condition medication. If you are in the Prevention Programme (PP), an annual review is required. Please request your prescription one week after your review appointment to allow processing time.

Registering for the programme

If you have one of the eligible conditions and hold a medical card or GP Visit Card, please contact us to discuss enrolment. Your GP will confirm your eligibility at your next appointment.

Your information

Information collected as part of your CDM care is held securely. The HSE may use anonymised data to improve chronic disease services and outcomes. Full data protection compliance is maintained in accordance with GDPR.

Opting out

Participation in the CDM programme is voluntary. If you wish to opt out, please speak to your GP or contact reception.
