Important information

A GP consultation is required for sick certificates and Illness Benefit certificates. Recent hospital documentation may be used in lieu of a consultation in some circumstances. Please allow 48 hours for processing.

Sick certificates

If you are unable to attend work due to illness or injury, your GP can issue a sick certificate. A consultation is required. Certificates are delivered by email within 2 working days.

For work-related absences, your employer may require you to submit a specific form — please bring this to your appointment if applicable.

Illness Benefit (IB1 Form)

If you are unable to work due to illness and wish to claim Illness Benefit from the Department of Social Protection, your GP must complete the IB1 form. Please note:

  • Pre-employment medical certificates
  • Insurance certificates and forms (Laya, VHI, etc.)
  • Driving licence medicals (Group 1 and Group 2)
  • Sports medicals
  • School / college medical forms
  • Disability and social welfare forms

For insurance forms, please allow up to 10 working days. A fee applies depending on the complexity of the form. See our fees page or call reception.
