Every letter is reviewed and must be approved by your GP before it is issued, so please do not leave requests until close to a deadline.

Our admin team are not permitted to accept letter requests over the phone or at reception. Please use one of the methods below.

Important conditions

GP review and approval

All letters are drafted by the practice team and reviewed by a GP before being issued. The GP may amend the wording, request more information from you, or decline the request if a consultation is needed first.

Not for sick certs or referrals

This form is for general support letters. Sick certificates, Illness Benefit forms and driving medicals have their own process — see Medical & Social Welfare Certificates.

Fees

An administration fee applies, from €30 depending on the complexity of the letter. See our fees page for details.
