An electronic check-in kiosk is available upon entry to reception. Please use it when you arrive for your appointment — it takes just a few seconds and helps our team manage waiting times efficiently.
If you need assistance using the kiosk, please ask a member of our reception team who will be happy to help.
Patients are required to notify the clinic at least 24 hours in advance if they need to cancel or reschedule their appointment. Cancellations can be made by phone or email.
Patients missing more than three appointments within 12 months may face discharge from the practice. Missed appointment slots cannot be offered to other patients in need of care.
Most viral infections in children do not require antibiotics and will resolve on their own. The HSE provides helpful guidance on managing coughs and colds at home.

Provides free GP visits, prescriptions, and hospital care for eligible patients. Apply online at hse.ie. Our reception team can assist with queries.
Children under 8 qualify automatically for a GP Visit Card providing free GP visits. Register through hse.ie. Bring your child's PPS number and proof of address to your first appointment.

All persons aged 70 and over qualify automatically. Apply through hse.ie. No means test required.
Entitles you to necessary healthcare when visiting EU/EEA countries and Switzerland. Apply free of charge at hse.ie/ehic.
To transfer to a new GP, complete a Change of GP form available from the HSE. Your new GP will request your records from your previous practice. Forms available at reception.

Free drugs and medicines for certain conditions including diabetes, epilepsy, and multiple sclerosis. Ask your GP if your condition qualifies.
If you are unable to work due to illness, your GP can complete the IB1 form for the Department of Social Protection. A consultation is required. Bring the form with you.
For people coming off Illness Benefit or Invalidity Pension who wish to return to work with a reduced capacity. Your GP will complete a medical assessment form. Book an appointment and bring the form.
For employees injured at work or with an occupational disease. Your GP can complete the medical section. Book an appointment and bring the relevant DSP form with you.
A monthly payment for a child aged under 16 with a severe disability requiring ongoing care and attention. Your GP completes the medical section. Book an appointment and bring the form.
Your GP can complete the medical section of the Disability Allowance application form. Book an appointment and bring the form with you.
If providing full-time care to a person in need, you may be eligible for Carer's Benefit. Forms available from reception or at gov.ie.
Your GP completes the medical section of the MB2 form. Book an appointment and bring the completed form from your employer or the DSP.
Claim tax relief on GP fees and medical expenses through Revenue's MedEx system at revenue.ie. We provide receipts on request.
For dental expenses that qualify for tax relief, your dentist completes the MED 2 form which you then submit to Revenue. Available at revenue.ie.
Required for the Disabled Drivers and Disabled Passengers Scheme. Your GP completes part of this form. Book an appointment and bring the application form.
We can complete Laya claim forms. Please allow up to 10 working days. Drop your completed form to reception with the relevant sections filled in. A fee applies.

Required for truck, bus, and other Group 2 licence categories. Includes a mandatory eyesight assessment — please note that a separate eyesight report from an optician is also required. Book a driving medical appointment in advance.
